Visit Staff Health

The Employment staff will work with you to set up your appointment with Staff Health/OccuMed. Below are instructions for your post-offer health assessment:

  • Print and complete the following forms and take them with you to your appointment:

  • Please arrive 10-15 minutes prior to your appointment.

  • Staff Health is located on the first floor of the Doctor's Office Building at 50 Doctor's Drive, Asheville, NC 28801.  Parking is available in the front of the building.

  • You will need to have your driver's license--or other photo identification, if no driver's license is available--as proof of identification for your drug screen.

  • If you are taking any medications, bring a list of those medications with you.

  • Bring a copy of your immunization records. If you do not have a copy, try contacting your former employer, your school, or your local health department to get the records.  If you still cannot obtain a copy, titers will be drawn and your immunization level will be checked.  Please note that there is a fee for drawing titers!  Please see this letter for more information.

  • A condition of employment is obtaining an annual mandatory flu vaccine during flu season.  If you've already had a flu shot this season, bring the documentation with you.  Otherwise, you will receive the vaccine during your health assessment.

  • If you wear glasses or contacts, bring them with you.

  • You may eat and drink before your appointment, just limit your fluids to 1-2 eight oz. beverages 1 hour prior to the appointment.

  • The entire appointment should last about 45-60 minutes.

  • If your start date exceeds 30 days from your initial drug screen, a repeat drug screen is required prior to attending General Orientation.  It is your responsibility to contact the Employment Coordinator at 213-4437 if you need to reschedule.

After your health assessment appointment, come by Human Resources with the following information:

  • Two forms of identification, one photo and one non-photo government ID. (There is information on the I-9 form which you will complete via the on-line onboarding forms that gives examples of documents that are acceptable to use.)

  • A voided check from your bank account; your first check will be mailed to your home and your second check will be directly deposited into your account. Direct Deposit is a required condition of employment.
